5、 一、工程概況與特點： 1、項目名稱：泗涇公園兩側(cè)改造工程（西小門、北小門改造） 2、建設單位：上海市松江區(qū)泗涇鎮(zhèn)村鎮(zhèn)建設管理辦公室 3、項目地址：鼓浪路 4、建設規(guī)模：建筑面積200m2 5、設計單位：上海建旗建筑工程設計有限公司 6、施工單位：上海泗涇建筑工程有限公司 7、工程說明： 本工程位于松江區(qū)鼓浪路，建筑面積200m2， 其中西小門、北小門門頭、門衛(wèi)；墻體±0.000以下為MU15混凝土實心磚， M10水泥砂漿砌筑；±0.000以上采用MU10混凝土多孔磚，M7.5混合砂漿砌筑；門頭鋼結(jié)構(gòu)屋面、門衛(wèi)鋼筋混凝土屋面。外門窗采用鋁合金門窗；中空玻璃均采用（5+9

6、A+5），其中衛(wèi)生間采用中空磨砂玻璃，窗單扇面積>1.5 m2的采用鋼化中空玻璃，門玻璃采用鋼化中空玻璃；工程計劃開竣工日期：2012年5月26日~2012年8月26日。 8、 工程安全生產(chǎn)重點部位： （1） 井架、起重機、等使用及裝、拆防護； （2） 吊裝區(qū)域防護； （3） 腳手架搭、拆安全防護； （4） 三寶、四口、五臨邊防護； （5） 模板工程支、拆防護； （6） 施工用電安全管理。 9、督促施工單位對以上危險源采取相應對策： （1） 建立施工現(xiàn)場危險作業(yè)管理制度； （2） 列出危險源清單； （3） 實施危險作業(yè)許可證制度； （4） 建立重點部位控制記錄；

7、 （5） 進行定期防火安全檢查和記錄； （6） 進行定期場容場貌與工地環(huán)境衛(wèi)生檢查和記錄。 二、編制安全監(jiān)理實施細則的依據(jù)： 1、已編制《監(jiān)理規(guī)劃》； 2、已編制《施工組織設計》； 3、《中華人民共和國安全生產(chǎn)法》； 4、《建設工程安全生產(chǎn)管理條例》； 5、《關(guān)于實行建設工程安全監(jiān)理制度的通知》（滬建建[2003]605號文） 6、《關(guān)于實施建設工程安全監(jiān)理的指導意見》（滬建建管[2003]170號文）； 7、《建設工程監(jiān)理規(guī)范》（GB50319-2000）； 8、《工程建設標準強制性條文—施工安全部分》（2002年版）； 9、《建筑施工安全檢查標準》（JGJ59-20

8、11）； 10、《施工現(xiàn)場臨時用電安全技術(shù)規(guī)范》（JGJ46-2005）； 11、《建筑施工高處作業(yè)安全技術(shù)規(guī)范》（JGJ 80-91）; 12、《建筑機械使用安全技術(shù)規(guī)程》（JGJ33-2001）； 13、《建筑施工扣件式鋼管腳手架安全技術(shù)規(guī)范》（JGJ130-2011）； 14、《龍門架及井架物料提升機安全技術(shù)規(guī)范》（JGJ88-92）； 15、《施工現(xiàn)場安全生產(chǎn)保證體系》（DGJ08-903-2010）。 三、安全監(jiān)理工作目標： 督促施工承包單位建立安全生產(chǎn)的保證體系和安全生產(chǎn)崗位責任制，督促施工方根據(jù)施工組織設計中的安全技術(shù)措施加以實施，協(xié)助承包單位完成自己提出

9、的工程安全管理目標： 1、杜絕死亡、火災等重大事故和食物中毒事故； 2、工地安全檢查按建設部JGJ59-2011標準達標； 3、工地安全保證體系按DBJ08-903-98標準貫標； 四、安全監(jiān)理工作的程序： 1. 項目總監(jiān)理工程師在編制項目監(jiān)理規(guī)劃時應明確開展現(xiàn)場安全監(jiān)理工作的目標及組織機構(gòu)，各級監(jiān)理人員安全監(jiān)理職責和權(quán)限、安全監(jiān)理內(nèi)容、安全監(jiān)理工作程序、安全監(jiān)理工作制度及措施，必要時應報建設單位備案。 2. 安全監(jiān)理人員應根據(jù)工程建設的實際情況，施工承包單位編制的施工組織設計中安全技術(shù)措施、安全生產(chǎn)保證計劃及本公司制訂“安全監(jiān)理管理辦法”的規(guī)定，在工程開

10、工前編制“安全監(jiān)理實施細則”，經(jīng)總監(jiān)理工程師審核批準后實施，必要時召開專題會議向施工承包單位進行交底。 3. 在安全監(jiān)理工作中，監(jiān)理人員學習有關(guān)安全生產(chǎn)規(guī)定、規(guī)范、標準、監(jiān)理委托合同和安全監(jiān)理實施細則等文件，及時傳達政府、公司和建設單位有關(guān)施工安全文件和會議精神，相互交流，提高安全監(jiān)理水平。 4.安全監(jiān)理人員按要求審查施工承包單位編制的各類安全施工方案，并收集與安全管理工作相關(guān)的“安全管理協(xié)議書”和“施工安全總交底記錄”，發(fā)現(xiàn)與法律、法規(guī)和安全施工強制性標準不符的應書面要求施工承包單位調(diào)整或補充。 5. 安全監(jiān)理人員必須在監(jiān)理日記的其他項內(nèi)填寫安全監(jiān)理工作內(nèi)容，包

11、括安全監(jiān)理工作、施工現(xiàn)場安全狀況、處理意見等，日記中涉及書面整改要求的應記錄相關(guān)文件的編號?？偙O(jiān)代表應每周不少于一次進行檢查，并簽署監(jiān)理日記。 6. 監(jiān)理項目部應與監(jiān)理工作月報同步編制“安全監(jiān)理工作月報表”，經(jīng)安全監(jiān)理人員簽署意見后，作為監(jiān)理工作月報的附件。 7. 安全監(jiān)理人員應每周不少于兩次對施工現(xiàn)場進行安全工作巡視，并按施工現(xiàn)場實際情況對照相關(guān)監(jiān)控要求逐項填寫“巡視旁站檢查記錄”。相關(guān)的安全監(jiān)理監(jiān)控要點應參照公司編寫的“安全監(jiān)理監(jiān)控要點”的規(guī)定執(zhí)行。遇有引發(fā)重大安全隱患的工序和部位、高危作業(yè)的關(guān)鍵工序?qū)嵤┈F(xiàn)場跟班監(jiān)督檢查和旁站監(jiān)理。發(fā)現(xiàn)重大安全隱患應及時向總監(jiān)理工程上匯報，

12、必要時監(jiān)理項目部應使用照相等手段正確記錄施工現(xiàn)場安全生產(chǎn)情況。“巡視旁站檢查記錄”應作為編制“安全監(jiān)理工作月報”和簽發(fā)“監(jiān)理工程師通知單”的依據(jù)之一。 8. 總監(jiān)應定期舉行安全監(jiān)理會議（可與工地例會合并召開），會議的主要內(nèi)容：檢查上次會議執(zhí)行情況，匯報施工人員、機具及現(xiàn)場施工安全狀況及必要的新議題，對發(fā)現(xiàn)的安全隱患應在會上確定整改措施和落實責任人員。由監(jiān)理項目部根據(jù)會議情況編寫“工地會議記錄”。在工地會議記錄中要反映上述安全內(nèi)容。 9. 在巡視旁站檢查過程中發(fā)現(xiàn)的安全隱患及違反“工程建設施工安全標準強制性條文”規(guī)定的情況，安全監(jiān)理人員應及時向施工承包單位開具“監(jiān)理工程師通知單”

13、（安全監(jiān)理），規(guī)定整改期限，并經(jīng)總監(jiān)簽字發(fā)出。在施工承包單位按通知單要求整改完畢后，安全監(jiān)理人員在接到“監(jiān)理工程師通知回復單”后應及時組織復驗，并由總監(jiān)簽署整改復驗意見。 10. 出現(xiàn)重大安全隱患（指可能直接影響工程結(jié)構(gòu)和人員生命安全）或未按“監(jiān)理工程師通知單”（安全監(jiān)理）的要求限期整改的情況，應由總監(jiān)下達工程暫停令，要求施工承包單位立即對指定部位停工整改。工程暫停令應及時抄送建設單位和監(jiān)理公司工程部負責人，必要時由工程部報送負責本工程的施工安全監(jiān)督部門。 11. 為加強施工現(xiàn)場的安全管理工作，總監(jiān)應以“監(jiān)理工作聯(lián)系單”（安全監(jiān)理）的形式向工程建設參建各方書面發(fā)出建議和意見，以

14、此加強相互間的溝通和協(xié)調(diào)工作。 12.對施工現(xiàn)場發(fā)生的傷亡事故，項目總監(jiān)應立即向監(jiān)理項目部及公司工程部報告。一般傷亡事故在24小時以內(nèi)，重大和特大傷亡事故在2小時內(nèi)向監(jiān)理項目部及公司工程部書面報告?zhèn)鍪鹿是闆r。具體內(nèi)容包括：簡要經(jīng)過、事故性質(zhì)、事故原因、人員傷亡情況、已采取的措施和監(jiān)理所做的相關(guān)工作等?，F(xiàn)場監(jiān)理項目組應對事故的處理過程和處理結(jié)果進行跟蹤檢查。 五、工程項目安全監(jiān)理工作方法與監(jiān)控要點： 1. 施工準備階段： 總監(jiān)理工程師在工程開工前，應組織全體人員對該項目的合同文件進行全面的了解和熟悉，了解現(xiàn)場的環(huán)境、人為障礙等因素，以便掌握障礙所在和不利環(huán)境影響的有關(guān)資料，

15、及早提出防范措施。 （1） 第一次工地會議： 為使施工承包單位在開始階段能和安全監(jiān)理人員建立良好的合作關(guān)系，使施工承包單位了解安全監(jiān)理人員關(guān)于督促和管理施工安全的程序和意見，了解各級安全監(jiān)理人員要履行的職責、權(quán)限等。 第一次工地會議向施工承包單位交底主要內(nèi)容和要求： a. 督促施工單位落實安全生產(chǎn)的保證體系，建立安全生產(chǎn)責任制； b. 要求明確安全生產(chǎn)網(wǎng)絡體系，做到各級人員責任明確、分工合理； c. 安全管理方針為安全第一，預防為主，科學管理，落實責任，嚴肅法規(guī)，杜絕違章； d. 要求制訂工程項目安全管理目標； e. 提供工程難點分析、危險源清單，并落實管理措施。 （2

16、）督促承包單位建立安全生產(chǎn)責任制： a. 確定安全生產(chǎn)領(lǐng)導小組和成員名單及領(lǐng)導小組的作用； b. 建立健全縱向到底、橫向到邊的安全生產(chǎn)責任制和群防群治制度，明確建立以項目經(jīng)理、各部門、各班組的安全生產(chǎn)組織機構(gòu)并制訂各自在安全管理網(wǎng)絡圖中的職責； c. 制定出安全生產(chǎn)獎懲考核制度及辦法。 （3）督促承包單位編制和有效實施安全生產(chǎn)保證計劃： a. 安全生產(chǎn)保證計劃編制有具體工程安全管理目標； b. 安全生產(chǎn)保證計劃應包含工程難點分析、工程重點部位及其控制手段； c. 列出符合工地實際情況的安全保證體系要素及職能分配表； d. 各要素實施具體落實到監(jiān)護人、責任人、檢查人、維修保

17、養(yǎng)人； e. 要求按照施工現(xiàn)場安全生產(chǎn)保證體系管理資料（工地安全管理臺帳），詳細填寫安1~安8有關(guān)安全生產(chǎn)保證體系資料； f. 制訂出施工現(xiàn)場安全文明控制要求及實施要點； g. 承包單位要做好安全檢查、糾正措施和預防措施。 （4）審查承包單位施工方案及安全技術(shù)措施，并督促其實施。 a. 審查承包單位施工組織設計中的安全技術(shù)措施及承包單位項目部、公司的審批意見、會簽、企業(yè)蓋章等手續(xù)是否齊全。施工組織設計中相關(guān)安全技術(shù)措施方案（如：臨時用電、井架、腳手架、）均要求單獨編制專項安全技術(shù)措施方案，在實施過程中對照安全技術(shù)措施方案進行驗收，各類驗收單安全監(jiān)理留存?zhèn)浒浮? b. 施工承包單位提供

18、的安全員及特種作業(yè)人員名冊及資格證書復印件包括安全員、電工、電焊工、架子工、井架搭拆工、中小型機械操作工等進行備案，要審查上崗證是否有效，是否超期，參照公司“安全監(jiān)理監(jiān)控要點”13.特種作業(yè)人員監(jiān)控要點執(zhí)行。 c. 施工承包單位進場大型施工機械設備，要求施工單位按批準的施工組織設計組織施工，并進行相關(guān)的報驗手續(xù)。 d. 中小型施工機械、施工用電的驗收要求施工單位按批準施工平面布置圖及施工臨時用電方案組織施工。 e. 安全監(jiān)理人員應根據(jù)建設單位與施工承包單位簽訂的合同及安全生產(chǎn)協(xié)議書約定，要求施工承包單位書面明確安全管理機構(gòu)和責任人員，協(xié)議的甲、乙方的法定代表人的簽章手續(xù)齊全，安全協(xié)議的復

19、印件安全監(jiān)理留存?zhèn)浒浮? f. 安全監(jiān)理人員督促檢查施工承包單位在項目部進場后做好安全生產(chǎn)總交底，交底后履行交底人與被交底人的簽字手續(xù)，監(jiān)理要保存相關(guān)交底記錄。 2. 基礎(chǔ)施工階段： （1） 樁基施工時應審查樁基施工承包合同、隊伍資質(zhì)情況，掌握承發(fā)包安全協(xié)議書中安全內(nèi)容。審核樁基施工方案內(nèi)涉及到操作安全規(guī)定的內(nèi)容。 （2） 基坑開挖中造成坍塌事故的主要原因： a. 基坑開挖放坡不夠； b. 施工方法不正確，開挖程序不對，超標高挖土； c. 支撐設置或拆除不正確； d. 排水措施不力以及解凍時造成的坍塌。 （3） 安全監(jiān)理人員督促施工承包單位按基坑施工方案實施，并按照“安全監(jiān)理監(jiān)

20、控要點”中1.基坑施工監(jiān)控要點執(zhí)行。 （4） 審核中小型施工機械驗收檢查情況，做到機械安全防護裝置齊全、完好，接地或接零保護符合要求，要求符合上崗條件機械操作人員嚴格按照機械安全操作規(guī)程操作。 （5） 檢查承包單位按臨時用電施工方案進行實施，按照現(xiàn)場平面布置圖督促施工單位采用TN-S系統(tǒng)，電纜需采用五芯電纜，配電系統(tǒng)采用“三級配電，兩級漏電保護”的措施。 3. 結(jié)構(gòu)施工階段： （1）安全監(jiān)理人員應檢查“三寶”“四口”的防護工作，督促施工承包單位做好臨邊、洞口等的防護工作。 a. 定期對施工單位報審“三寶”“四口”防護檢查評分表進行審核； b. 在施工作業(yè)面邊沿無防護設施或圍護設施高

21、度低于80cm時，都應要求施工單位按規(guī)定搭設臨邊防護欄桿； c. 同時要求對在建工程地區(qū)入口處，施工現(xiàn)場人員流動密集的通道上方按規(guī)定設置防護隔離棚，防止因落物產(chǎn)生物體打擊事故。 （2）督促施工單位必須按照“腳手架搭設施工方案”實施腳手架搭拆工作，腳手架基本形式要符合《建筑施工扣件式鋼管腳手架安全規(guī)程》JGJ130-2001構(gòu)造要求： a. 立桿模距1.00m，立桿縱距1.80m，內(nèi)立桿距墻0.3m，步距1.80m，連墻件設置二步三跨； b. 腳手架的地基、材質(zhì)、通則、立桿、大橫桿、小橫桿、連墻件、剪刀撐、掃地桿、斜道、腳手笆、防護欄桿、扣件安裝要嚴格按照“腳手架搭設施工方案”中的搭設具

22、體要求實施； c. 督促施工單位按照施工方案做好腳手架分階段檢查與驗收工作，拆除時要督促施工單位遵照施工方案中的拆除前的準備工作和拆除時應符合的規(guī)定進行工作。 （3）督促施工單位嚴格按照安全生產(chǎn)保證計劃中的對施工井架進行安全要點控制： a. 施工井架在裝、拆、修理、保養(yǎng)過程中的安全必須先制定方案，作業(yè)人員經(jīng)過培訓取得安全資格證，安裝后經(jīng)公司驗收主管部門檢測合格后，方可掛牌使用； b. 操作人員每班使用前按規(guī)定檢查制動、各限位裝置、井架籠門和圍護門等處的電器聯(lián)鎖裝置是否靈敏可靠； c. 監(jiān)理人員尚需按本公司編制“安全監(jiān)理監(jiān)控要點”中的6.井架搭設監(jiān)控要點對其他安全監(jiān)控點加以檢查和督促。

23、 （4）督促施工承包單位做好模板工程安全生產(chǎn)工作，要求按照模板施工方案中的安全技術(shù)措施及安全生產(chǎn)保證計劃中的模板工程安全措施執(zhí)行。 a. 模板工程必須有安全技術(shù)措施的施工方案，并經(jīng)上級主管部門審批； b. 進行模板施工時，施工人員必須戴好勞動保護用品并系牢； c. 工作前應先檢查工具是否牢固，釘錘、扳手等工具是否用繩鏈系在身上，用后應放入工具袋內(nèi)，以免掉落傷人； d. 高空作業(yè)安裝與拆除模板，應加強安全防護措施，必要時須搭設腳手架并設防護欄桿； e. 盡量避免在同一垂直面上下同時操作； f. 安裝模板所用的接頭、撐頭必須釘牢，支撐必須穩(wěn)固可靠，對散松等異常情況時應及時整修，嚴防倒

24、塌事故； g. 支模應按順序進行，模板及支撐系統(tǒng)在未固定前，禁止利用欄桿、支撐作攀登； h. 模板的拆除必須符合混凝土的設計強度； i. 拆模板時禁止使用木料及鋼模板作立人板。 （5）督促施工承包單位按臨時用電施工方案嚴格執(zhí)行： a. 所有電纜采用埋地，臨時用電線采用架空，以確保安全； b. 電工須經(jīng)有關(guān)部門培訓考試合格，持有效證件操作； c. 值班現(xiàn)場電工每天做好用電設備巡視檢查，并做好記錄； d. 電工必須每月不少于2次對配電箱內(nèi)漏電保護器等電氣設備與線路進行檢查，檢查表貼在配電箱內(nèi)； e. 每季度對各種電箱和各種設備進行接地接零電阻絕緣測試記錄歸檔； f. 移動機具及

25、照明用電實行二級漏電保護，電工應經(jīng)常檢查、維修和保養(yǎng)； g. 安全監(jiān)理巡視檢查工作按“安全監(jiān)理監(jiān)控要點”3.、“施工用電監(jiān)控要點”執(zhí)行。 （6）督促施工承包單位按安全生產(chǎn)保證計劃加強對中小型機械安全管理： a. 由機管員負責機械使用前驗收工作及機械日常檢查工作； b. 操作人員必須持有效證件上崗，按安全操作規(guī)程實施操作； c. 按規(guī)定搭設機械防護棚； d. 機械設備必須接地或接零，隨機開關(guān)靈敏可靠，安全防護裝置齊全有效，嚴禁帶病運轉(zhuǎn)，所有電動工具必須做到用電二級保護； e. 督促操作人員做好機械檢查、保養(yǎng)及維修工作； f. 安全監(jiān)理人員應根據(jù)“安全監(jiān)理監(jiān)控要點”中10.施工機具
